Default My Letter to the insurance

Please tell me what you all think.......Its long and I know its not all grammatically correct

Diet History of Rebecca Sellers
1984 – 12 years of age, Mother put me on Herbal Life Diet Pills and Shakes and lost 60 pounds. Kept the 60 pounds off until 1989.

1989- Regained the 60 pounds and tried the Cabbage Soup Diet. Lost 30 pounds. Quit the diet because of the awful taste and side effects associated with it. Regained the 30 pounds within the year.

1990- At my all time high weight of 180 pounds at 5ft 2 in. Tried the slim fast diet. Lost 15 pounds. Couldn’t afford it on a student’s salary and quit. Regained the 15 pounds plus 5 more.

1991 – Tried the efedra diet pills over the counter. Lost 20 pounds but they made me shake and feel jittery so I quit taking them. Kept those 20 pounds off for 3 years.

1994 – Regained about 20 pounds. Tried the grapefruit diet where you eat grape fruit for breakfast and drink grapefruit juice for lunch and then eat a dinner. Lost 10 pounds. It was such a strict diet that I quit. Of course I regained the 20 pounds.

1995- Became pregnant gained from 185 pounds up to 230 pounds.

1996 -Gave birth and lost down to 214 pounds. Stayed at that weight until I became pregnant with my second child.

1997-I developed Gestational diabetes and maintained a weight of 214 throughout my pregnancy. I gave birth and lost down to 172 pounds. The smallest I had been in years. Moved to Germany with my husband and children and within a year I had gained 43 pounds. I went to a German doctor and he prescribed me diet drops (Equivalent to Efedra). I lost 30 pounds but when I quit taking them gained back all 30 pounds.

1998- Didn’t really try any diets had kind of given up.

1999- Tried Fat Burners and a low calorie diet. Lost 30 pounds kept it off for a year.

2000- Gained back the 30 pounds I lost the year prior. Tried doing a low fat diet with exercise. Lost 30 pounds. Kept the 30 pounds off for a year.
2001- Became pregnant with my 3rd child. Gained up to 250 pounds. I delivered in 2002 and only lost 20 pounds.

2002-Hubby went to Iraq for 1st time and I did the Michael Thurmond 6 week body-make over. Lost 50 pounds. Kept it off until 2005. The diet is very restrictive, no salt and only certain kinds of food combo’s.

2005 -decided to try and get approved for gastric bypass. I have done research on this for over 4 years before actually deciding to take the steps.

2006- My PCM referred me and here I am now.

I currently have Type 2 diabetes and I’m on Glucofage. I also have acid reflex do to my size. I have a hard time keeping up with my three children because they are so active and I am unable to keep up with their activity level. I am afraid to take my four year old to the park because if he were to run out in the street I couldn’t catch him before he got into trouble. I also suffer from anxiety. I feel like people are talking about me when I am in public. My father passed away when I was 20 years old. He was only 39. He died from a stroke caused by high blood pressure. I have in the past been on medication for hypertension. Just recently taken off because of the weight I have lost in the program here at Queens Comprehensive Weight Management. I have three young children and would like to live a long and healthy life for them. I feel that with the way I am going if I don’t lose this weight forever I will join my dad in an early grave.

I have chosen Gastric Bypass as a last resort to this life of obesity.
I feel like I have the knowledge and determination and the support to succeed at this. Queens Comprehensive Weight Management Program is an awesome program. You are seen by your psychologist several times to help you through issues you may have. You are also taught extensively by the dieticians on staff. You don’t just see them once you see them all over and over until you have the knowledge you need to make this tool a success. Gastric Bypass is a tool given to us and it is up to us to make it work. If we aren’t committed to eating healthy and exercise then we will not be successful. I feel I am ready to use this tool and become a success. I know this is a lifetime change and I am ready for it. I also am happy to have the Queens CWMP because I know if I need help all I need to do is call and they will guide me.

Alt Weight Loss Surgery Insurance
Yes, you can often get insurance cover for your Weight Loss Surgery.
Our own Craig "Big-T" Thompson has been there and done that, and he's written an e-book about it.
